Clarity Chiropractic (“Clarity Chiropractic,” “the Clinic,” “we,” “us,” or “our”) understands the importance of privacy and is committed to maintaining the confidentiality of your medical information.
We create and maintain records of the health care services we provide to you and may receive medical records and information from other health care providers. We use these records to provide and coordinate your care, allow other health care providers to provide appropriate care, obtain payment for services provided to you as permitted by law, and meet our professional, administrative, and legal obligations in operating our practice.
We are required by law to provide you with this Notice explaining Clarity Chiropractic’s privacy practices regarding your medical information and how we may use and disclose your protected health information (“PHI”) for treatment, payment, health care operations, and other purposes permitted or required by law.
You have certain rights regarding the privacy of your protected health information, which are also described in this Notice.
We are required by law to maintain the privacy of your medical information and to provide you with this Notice of our legal duties and privacy practices. A current copy of this Notice will be made available to you upon request.
We are required to abide by the terms of the Notice currently in effect. Clarity Chiropractic reserves the right to revise this Notice and make the revised provisions effective for all PHI we maintain. If Clarity Chiropractic makes a material change to this Notice, the updated Notice will be made available through our office and, when applicable, posted on our website.
What Is Protected Health Information?

We may use and disclose your protected health information to provide, coordinate, or manage your health care and related services.
We may disclose your health information to other health care providers who are involved in your treatment or who have been requested to participate in your care.
For example, we may share relevant information with referring physicians, your primary care provider, medical specialists, imaging facilities, physical therapists, pain management providers, pharmacies, or other health care professionals involved in your care.
Payment
We may use or disclose protected health information as necessary to obtain payment for services provided to you and to carry out billing and payment-related activities permitted by law.
We may also disclose relevant PHI to family members or other individuals you identify when that information is directly relevant to the individual’s involvement in your care or payment for your health care.
